FEE POLICY FOR SHRUTHI SWARA LAYA

Fees:

Beginner:	$90 / month (Four classes)
Intermediate:	$110 / month (Four classes)
Advanced:	$135 / month (Four classes)
One-on-One:	$150 / month (Four classes)
Workshop:	$40 /session
Arangetram (Solo Debut - formal on stage):	Please call for details

Classes are usually held during weekends. Teachers are allowed vacation which they will inform the students at appropriate times. These classes are compensated during months where a 5th (week / weekend) class is possible.

The exact dates will be conveyed by the respective teachers.

There will be a $25 registration fee for all new admissions.

National holidays and a few 'Indian' traditional holidays (ex: Deepavalli) will be in force. It is left to respective teachers to conduct any such 'missed' sessions in a manner that is most suitable for the class.

Fee Policy:

1.	Fee is due every first class of the month.
2.	Fee can be deposited in the cash box, handed over to the teacher or at the front desk.
3.	There will be $25 fine for late payment.
4.	In the case of absence, please mail the check to SHRUTHI SWARA LAYA on or before the first class of the month.
5.	Any vacation schedule must be informed to teacher at least four weeks prior to absence, failing which the regular fee will be charged.
6.	Full fee amounts are due even if the student is absent for any session during the month - prorating of fees is not permissible.
7.	Special terms:
7.1.	For Karnatic Vocal enrollment: This contract between student (guardian / parent) and Shruthi Swara Laya is for a contiguous period of on year from date of sign off. For Karnatic Vocal enrollment: Any termination during the above contracted period entails complete one time payment for the remaining contractual period based on the above rate with an average of four classes per month.

Please read the above fee regulations carefully and sign the agreement below. There will be no exceptions.

CLAIMS WAIVER - SHRUTHI SWARA LAYA

Due to exorbitant rise in insurance premiums, we find ourselves unable to pay for general insurance, which services of this nature demand. We are approaching you for a waiver as follows:

I, on behalf of myself or as a parent or legal guardian of the student(s) obtaining the services of Shruthi Swara Laya or its parent organization Vimal Orbits Inc.,

on behalf of ourselves or the above mentioned student(s), hereby waive all rights, claims and actions, which we, now or hereafter may have against the aforementioned organizations, its faculty or its staff, arising out of, but not limited to the above mentioned student's attendance, and/or due to the use of the facility at the premises where the businesses are conducted.

I have read and fully understood the meaning and consequences of the foregoing waiver and thus voluntarily execute this waiver accordingly.
